The receptionist will need to make a photocopy of your insurance card. We ask that you come prepared to pay your co-pay and/or deductible and we will file your insurance for the exam.
A refraction is the exam to determine the glasses power. It is, unfortunately, a non-covered procedure by Medicare and many other insurances. You may be responsible for a $31.00 charge if a refraction is done.
If you wear disposable contact lenses, please bring your contact lens prescription or your contact lens foil packs, boxes, or bottles, even if they are empty.
If you are using prescription medications, it is suggested that you bring those with you so that we may record this information in your medical chart.
